Plymouth Family Physicians Named a 2017 Million Hearts® Hypertension Control Champion

Healthy Hearts in the Heartland (H3) congratulates the primary care practices recognized as 2017 Million Hearts^® Hypertension Control Champions. Among the 24 honorees is Plymouth Family Physicians (Plymouth, WI), a practice participating in H3.

Each of the Champions – primary care practices, individual clinicians, and health systems – is being recognized for achieving blood pressure control for at least 70% of their patients through innovations in health information technology and electronic health records, patient communication, and health care team approaches.

H3 is part of the Agency for Healthcare Research and Quality’s EvidenceNOW initiative to advance heart health in primary care. Through EvidenceNOW, H3 is helping small- and medium-sized primary care practices use evidence to control patients’ blood pressure and address other risk factors to improve heart health.
