Hidden Cost: The Overlooked Merit-based Incentive Payment System (MIPS) Category
In 2017, the MIPS Cost category was collected but not scored. In 2018, Cost measures were weighted at 10% of final score but largely limited to primary care. In 2019, the category weight increased to 15% and Medicare introduced “episode-based” measures to broaden the scope of affected clinicians. While CMS has held the category weight at 15% for 2020 as part of COVID-19 relief, statute requires an increase to 30% by program year 2022.
In spite of this trend reflecting Medicare’s increasing attention, MIPS Cost continues to be overlooked due to reasons such as: no submission requirements (Medicare calculates automatically), lack of real-time data/feedback, and uncertainty on how to improve.
