Medicaid AIU Attestation
(Adoption, Implementation, and Upgrading of EHR)
- Up-to-date HFS Provider Information Sheet — If sheet can’t be located, request it from the HFS Provider Participation Unit at 217-782-0538
(Illinois Department of Healthcare and Family Services: HFS) - Completed CHITREC’s HFS’ Medicaid encounter volume worksheet
- Username/Password for the MEDI system, which is a subsection of the myHFS website
Medicare AIU Attestation
- Printout of successful 90-day Meaningful Use and Clinical Quality Measure reports
- Electronic Health Record (EHR) information — Vendor, product name and version number
